F-89 FLSD Application Checklist & Required Documents
Use this checklist to organize the major application, exam and document steps in the FDNY F-89 Fire and Life Safety Director process.
F-89 Checklist at a Glance
- Complete the required 31-hour FLSD course with an FDNY-certified school.
- Keep your latest FLSD school certification/letter.
- Apply for and pass the N-85 computer-based exam.
- Prepare the FLSD application form and required verification/supporting documents.
- Apply for and pass the Z-89 computer-based exam.
- After obtaining the Certificate of Completion, have the building’s authorized representative request the onsite exam.

FDNY currently lists the latest 31-hour FLSD course school certification/letter as a supporting document for the N-85 exam application.
Documents commonly required for the Z-89 step
FDNY currently lists the following supporting documents for the Z-89 stage:
- FLSD application form
- FLSD verification letter, or an accepted prior/related Certificate of Fitness card where applicable
- N-85 passing letter
- Latest 31-hour FLSD course school certification/letter
Who schedules the F-89 onsite exam?
Candidates do not schedule the onsite exam themselves. FDNY states that only authorized personnel designated by the building owner may request the onsite F-89 or T-89 exam after the candidate has passed the N-85 and Z-89 exams and obtained the Certificate of Completion.
